FSS Assessor/Counselor Outpatient

Confront has an immediate opening for a FFS outpatient Drug and Alcohol treatment counselor/assessor to join our multidisciplinary team. Applicant must meet DOH DDAP Licensing Standards for the position of counselor, CADC and/or experience with co-occurring populations preferred. Knowledge of approaches to trauma informed care.

Educational requirement: Bachelor’s degree from an accredited university in a clinical mental health discipline such as Clinical Psychology, Counseling, Clinical Social Work, ASAM trained. Spanish speaking a plus. Veterans welcome. EOE

Case Manager - Northampton County

The Treatment Trends, Inc Case Manager will assist individuals in the community in accessing appropriate services to address substance use disorder. Case Manager will conduct level of care and utilize resources to place client in appropriate level of care and conduct a warm hand-off when applicable. Case Manager may carry a case load of clients in the community requiring services other than level of care to support individuals in all phases of treatment and recovery. The Case Manager will support community-based efforts to provide access to treatment, through collaboration with organizations and agencies in Northampton County. The Case Manager will assess client’s individualized needs and coordinate care across multiple health and social service systems, ensuring continuity of among the array of services and systems. Case Manager will be able to complete and submit all reporting and documentation as needed.

A minimum Bachelor’s degree in related field is required; a Master’s Degree in related field is preferred. It is strongly desired the individual have 4 or more years' experience and/or education in the behavioral health field. Experience in providing case management is strongly desired. Licensed Social Workers and Certified Addiction Specialists are encouraged to apply. Bilingual/Multilingual applicants are strongly encouraged to apply.

Case Manager - HERO Program - Northampton County

Treatment Trends seeks a full-time Case Manager for Northampton County’s Housing Emergency Response to Opioids (HERO) program. The goal of the program is eliminating barriers to treatment and housing for individuals with Opioid Use Disorder. Rental Assistance is available to those who meet criteria and remain engaged in the HERO Program.

The Case Manager oversees all aspects of care. The Case Manager completes an initial assessment, works with the client to develop an Individual Service Plan (ISP), and assists the client to obtain benefits and various recovery related resources. The Case Manager helps clients to meet the goals of the program by making appropriate referrals so clients can access the services they need. The Case Manager will be able to complete and submit all reporting and documentation as needed.

A minimum Bachelor’s degree in related field is required; a Master’s Degree in related field is preferred. It is strongly desired the individual have 4 or more years' experience and/or education in the behavioral health field. Experience in providing case management is strongly desired.
